Selbsauskunftsbogen?

hallo, bin noch neu hier, daher vielleicht ein paar überflüssige fragen..

ich habe über www.kur.org mir die ganzen formulare ausdrucken lassen. dabei ist auch ein 6 seitiger selbsauskunftsbogen für mich (inkl.2seiten für meine kinder).
jetzt lese ich hier immer wieder, dass ihr selbst an die 5-6 seiten dazuschreibt und dem antrag beilegt. reicht dieser bogen vom müttergenesungswerk den nicht aus? dort muss ich sachen ankreuzen oder beantworten/schildern, dann sind 2-3 linien dafür vorgesehen.
trotzdem extra noch was schreiben?

Hallo Du,
dieser Selbstauskunftsbogen fällt je nach KK unterschiedlich knapp aus.

Ich habe sowohl bei der letzten, als auch bei dieser Kurbeantragung ( beide ohne Kurberatungsstelle ) einige Seiten persönliches Anschreiben hinzugefügt, da dieser Ankreuzbogen mit ein bis zwei Zeilen meine Situation nicht treffend beschreibt.

Damit sich die KK einen guten Überblick über Deine persönliche Lage, Deine Erkrankungen und Deine mütterspezifischen Belastungen machen kann, empfehle ich Dir, auf jeden Fall ein persönliches Anschreiben beizulegen.

vielen dank für deine antwort. dann werde ich auf jeden fall auch nochmal was extra schreiben. dann schreibe ich halt in die jeweiligen zweilen z.b. s.nr.1 auf extra blatt oder so

Hallo maedelsmami

Ich habe meine Kur mit Hilfe der Diakonie gestellt . Meine Beraterin meinte ich solle noch meine Probleme und was so in den letzten 2-3 Jahren alles so Passiert ist aufschreiben . Ich habe dann alles Aufgeschrieben und hatte 6 Seiten voll . Bei meinem nächsten Termin hatte ich alles mit genommen und ihr gegeben und sie meinte es wäre gut aber zu lang . So hat sie mir geholfen das ich mich kürzer gefasst habe und am Ende waren es dann noch 4 Seiten . Sie meinte das es genug wäre sonst würde es vielleicht nicht ganz gelesen werden würde .
Ich würde auf jeden Fall eine Selbstauskunft von dir beilegen .
