ich hatte bei meiner Krankenkasse auch immer nur ein sehr knappen Selbstauskunftbogen dabei, nach Alltag etc. wurde da überhaupt nicht gefragt. Deshalb habe ich bei beiden Anträgen ein extra Schreiben für den MDK beigelegt, in dem ich geschildert habe, welche Schwierigkeiten zu meinem Kurwunsch geführt haben und was ich mir von der Kur verspreche.

Ich finde vor allem den Selbstauskunftbogen wichtig, denn wo sonst willst Du Deine Situation und Probleme schildern? Aber wenn Du kein Formular hast, ist das Formular vom MGW immer okay und ansonsten frei schreiben. Alles Gute!

Ich hoffe, es klappt. Meinen genauen Tagesablauf hab ich jetzt nicht mitdazugegeben, weil meine Probleme mitunter psychischer Natur sind, da hilft das nicht so viel denke ich.

Also dieser vorgedruckte Selbstauskunftsbogen, den du da jetzt als Link eingestellt hast, der muss sowieso und generell und überhaupt bei einem Antrag dabei sein. Den gibt es in verschiedenen Formen bei deiner Kasse oder wie dieser hier vom MGW.
Allerdings ist es schon besser, wenn man dazu noch schriftlich wenigstens die Länge einer A4 Seite seine Umstände, Tagesabläufe, Probleme ect. schildert.

Wenn du das jetzt doch nicht gemacht hast, dann kannst du es ja, falls es dazu kommt, das im Widerspruchsschreiben ausführlich tun...

Ich hatte auch "nur" die Atteste und den Selbstauskunftsbogen beiliegen.

Dazu hab ich für vorne dran noch ein kurzes Anschreiben gelegt.

Sehr geehrte Damen und Herren,

hiermit beantrage ich eine Mutter- Kind- Vorsorgekur nach § blabla.
Dann kurz geschrieben warum und am Ende noch die Klinik und den Wunschzeitraum eingefügt.


Es wurde innerhalb von 6 Tagen "meine" Klinik bewilligt und sogar mit dem Zeitraum hat es geklappt.
